Waiver of Liability
I/We, the undersigned, hereby certify that I/we are the parent or legal guardian of the participating athlete. I hereby give permission to the Head Coach and other instructors to seek appropriate medical attention in the event of an accident, injury, or illness. I will be responsible for all costs of medical attention and treatment.
I/We, the undersigned, for ourselves, our heirs, executors or administrators, waive, release, and forever discharge the Head Coach, instructors and staff, officers, agents, employees, representatives, successors and assigns of the Harrison Hoya Football Booster Club, of and from any and all rights and claims for damages, injury or loss to person or property, which may be sustained or occur during participation in speed and agility training or programs conducted at Harrison High School, whether or not the loss, injury, or damage occurs due to negligence.
I/We acknowledge that our child is physically fit and mentally capable of participating in athletic activities.
